St Basils has an opportunity for a Transition Coordinator to be based in our new Transition Hub based in the Bordesley Green area of Birmingham on a full time, permanent basis.  This is an excellent opportunity to work in a groundbreaking Transition Service with young people who have complex Mental Health and multiple needs. You will be assisting young people who are living in the community to move into the Transition Hub.

 

Salary: £22,824 – £24,350 PER ANNUM PRO RATA

St Basils works with young people aged 16-25 who are homeless or at risk of homelessness, helping over 5000 young people per year across the West Midlands region with specific services in Birmingham, Sandwell, Solihull, Walsall, Warwickshire, Worcestershire, the Wyre Forest and Coventry. This is a fantastic opportunity to help support the needs of young people within a new St Basils service.

 

Key activities specific to the role include:

  • To manage a small case load of young people with very complex needs; contribute to their comprehensive, formulation-driven assessment, in addition to developing dynamic, strengths-based support plans.
  • Ensuring the needs, wants and wishes of the young person are at the heart of their support plan by engaging them in a collaborative process, and finding creative ways to ensure that they are empowered to contribute to their own recovery.
  • To work closely with colleagues within the Transition Hub multi-disciplinary team to secure positive outcomes for young people; as well as developing positive relationships with the Young Persons’ friends, family and wider support network of professionals.
  • To support young people to maintain their accommodation, as well as providing consistent opportunities for the residents to take part in the running of the Transition Hub and contribute to service improvement.

 

The ideal candidate will hold:

  • NVQ 4 in Care, Advice and Guidance or other relevant subject
  • Experience of working within the following areas: Youth Work, Residential or Social Work, Probation/Criminal justice, Community Work, Education and Training, housing or support work.
