Transition Coordinator – Transition Hub

St Basils has an opportunity for a Transition Coordinator to be based in our Transition Hub based in the Bordesley Green area of Birmingham on a full time, permanent basis.  This is an excellent opportunity to work in a groundbreaking Transition Service with young people who have complex Mental Health and multiple needs. You will be assisting young people who are living in the community to move into the Transition Hub.

 

Salary: £23,566 – £25,141 per annum

This role requires expert knowledge and experience of working with vulnerable people as well as a core understanding of issues surrounding youth homelessness such as mental health, domestic violence and care leavers.

  • Experience of working within the following areas: Youth Work, Residential or Social Work, Probation/Criminal justice, Community Work, Education and Training, housing or support work.
  • NVQ Level 4 or equivalent in Social Care (or prepared to work towards)
